Student Registration - 5 easy steps (All referenced documents are on this page as pdfs.)
1. Email the names of students participating to Clint Funk ([email protected]) by February-1st (all e-mails—no paper). A. Names of participating students should be alphabetized by last name (separate lists for ES, MS, & HS), again, by e-mail - no paper). B. Please indicate ES, MS, or HS for each list of students (by e-mail—no paper). C. The director’s name, name of school and name of school district should also be included with each list (by e-mail—no paper). 2. Print a copy of the DCMEA Choir Registration and Invoice to include with your check (paper copy). All checks should be mailed by March 1 to Kristen Zlogar Middletown Area High School 1155 N Union St. Middletown, PA 17057 3. Print a copy of the DCMEA Choir Festival Student Registration and Agreement Form for each participating student (hard copies). Please include these with your check. Again, the check, the “registration invoice” and the “DCMEA Choir Festival Student Registration and Agreement Forms” should be mailed by March 1 to Kristen Zlogar Middletown Area High School 1155 N Union St. Middletown, PA 17057 4. Teachers should bring a completed DCMEA Student Registration and Medical Information form for each participating student to the festival (hard copies on paper); these forms do not need to be submitted in advance. Teachers should keep these hard copies with them during the festival. This form is below. 5. Teachers should bring a completed DCMEA Medication Administration Record for each participating student who will need to have medication(s) administered during the festival. Teachers should keep these hard copies with them during the festival. |
